In this office, Dr. Mark M. Buzaglo acts as the Privacy Information Officer. Do not hesitate to discuss our policies with him or any member of our office staff.
Unless specifically required under the law, we shall not disclose your personal information without your consent. In particular, our office will not supply your insurer with your confidential medical history. In the event this kind of a request is made, we will forward the information directly to you for your consent.
